Job Title: RN Admission Specialist – Home Health

Company: Home Health Care, Inc.

Compensation: $150 - $200 per visit + mileage reimbursement!

Take charge of the start of care journey. As an RN Admission Specialist with Home Health Care, Inc. , you’ll deliver expert OASIS documentation and launch personalized care plans for clients transitioning into home-based care.

Responsibilities:

• Perform comprehensive patient assessments and OASIS documentation
  
• Initiate individualized care plans
  
• Recommend and coordinate additional services
  
• Communicate with providers and caregivers to ensure safe transitions
  

Qualifications:

• Minnesota RN license in good standing
  
• Admission and OASIS experience
  
• Strong communication and time management
  
• Reliable transportation
